Whilst it is always possible to call on the same day and I will do my best to accommodate you, it is advisable to call at least a week in advance in order to ensure the best choice of appointment times. At the time of booking a credit card is required to secure your appointment.
When you arrive for your appointment you will be asked to complete a short form which gathers information about your health history and the reason for your visit. All information is confidential. If you would prefer to complete the form in advance you can download and print it by clicking on the links below. Please complete the "Health Information" and "Office Policies" pages and bring them with you to your appointment. If you have a recent diagnosis of cancer and/or have recently been treated for cancer, please take the time to complete the additional Medical Information form as well. Thank you.
