Convenience and ease of scheduling online or by phone
For your convenience you can now schedule your appointments online any time of day or night. Clicking the "Book Now" button below will take you to Genbook's secure website where you can proceed to view available times, enter your information and schedule a time that is right for you. You will need a valid credit card to secure the appointment. Once the appointment is confirmed you will receive a confirmation email as well as a reminder email 24 hours prior to your appointment. You will have the option of canceling the appointment via a link on the confirmation email up to 24 hours prior to your scheduled time. At the present time online appointments can be scheduled up to one month in advance.
Please note that outcalls and 30 minute massages can presently not be scheduled online. To schedule these and to schedule online appointments more than one month in advance please call (302) 226 8833.
Intake Information
When you arrive for your appointment you will be asked to complete a short form which gathers information about your health history and the reason for your visit. All information is confidential. If you would prefer to complete the form in advance you can download and print it by clicking on the links below. Please complete the "Health Information" and "Office Policies" pages and bring them with you to your appointment. If you have a recent diagnosis of cancer and/or have recently been treated for cancer, please take the time to complete the additional Medical Information form as well. Thank you.
